Dr Doireann O'Leary on the triggers that cause acne in women way past their teens

Acne is a common skin condition that affects one in ten globally. Not restricted to the teenage years, it occurs in older women's skin often because of hormones.

To discover more about the condition, its causes and management strategies we spoke with lecturer and Cork GP, Dr Doireann O'Leary.

Acne isn't simply a case of having a breakout regardless of severity, it seems, and the sufferer can take some steps to help manage it at home.

'Acne isn't just skin-deep; it's a multifaceted issue involving factors like an excess buildup of skin cells within hair follicles, overproduction of sebum (oil), bacteria, and inflammation,' explains Dr Doireann.

'It most commonly appears on hormonally sensitive areas like the face, neck, and upper back - which of course presents its own challenges, she says. 'Beyond physical symptoms, acne can take a toll on mental well-being. Issues like low self-esteem and anxiety are common, which highlights the importance of holistic care and support.'

It's understandable how distressing this would be for a young person, but as it affects older women too, what exactly is at play there?

Dr Doireann says that 'Adult acne in women, often triggered by hormonal shifts, can wreak havoc on the skin. From menstrual cycle fluctuations to pregnancy-related hormonal changes and conditions like polycystic ovary syndrome (PCOS), understanding these triggers is really important for effective management.'

Some of the acne triggers are diverse, with tight clothing, pore-clogging cosmetics, medication and diet featuring heavily.

Certainly, anyone with acne should only ever use non-comedogenic skin, sun and makeup and get into a very good skincare routine every morning and evening that might include a low-strength salicylic acid cleanser or toner to start with.

At-Home Care

SPF sunscreen is another non-negotiable daily habit, especially if using retinoids for the skin condition. UV rays can cause acne scars to darken and the retinoid can increase skin sensitivity to sunlight.

Finding out diet triggers might require advice from a GP or skin professional so that is another route to take. About clothing, cotton and linen are breathable and looser, and make sure to change bed linen every couple of days, pillowcases especially where excess oil and bacteria can gather.

For severe cases, she says, 'oral medications like antibiotics or retinoids may be prescribed by your GP. Although they’re effective, they do require close medical supervision.'

But, is there any truth to that age-old toothpaste myth? 'While toothpaste may seem like a quick fix, it can actually irritate the skin,' she cautions. 'Effective, safer treatments are available through healthcare professionals.'
