‘Stop work’ order after pipes crush lorry driver to death
Penang DOSH says it carried out an initial investigation when it was informed that six ‘hollow pipes’ had fallen on the lorry driver at a factory in Simpang Ampat on April 29.
BUKIT MERTAJAM: The Penang department of occupational safety and health (DOSH) has issued an immediate “stop work” order on the loading and unloading of pipes at a factory in Simpang Ampat, near here, after a lorry driver was crushed to death by falling pipes.
Its director, Hairozie Asri, said the department has also instructed the employer to conduct an internal investigation into the cause of the incident, and identify measures to improve safety.
The company has been asked to put up a “case presentation” to the department.
Hairozie said a DOSH team conducted a preliminary investigation after being informed of the incident on April 29, in which a 60-year-old lorry driver died when six “hollow pipes” weighing more than 5,000kg fell on him.
He said the department will carry out a follow-up investigation by obtaining documents and identifying witnesses as well as making further visits to the factory.
