Une opération pédiatrique du rein

Un encadrement plus serré de la chirurgie pédiatrique et le manque de médecins fait craindre des années difficiles. Des déboires qui concernent surtout l’hôpital public, le secteur privé étant peu impliqué dans ce type de chirurgie.

D’un côté, les chirurgiens pédiatriques se félicitent que leur spécialité soit mieux reconnue et qu’un nouveau régime d’autorisations encadre mieux la pratique (en se basant notamment sur un nombre d’actes annuel). De l’autre, ils craignent que « l’on détricote l’existant » avant qu’une nouvelle organisation ne soit stabilisée.

Et ils s’alarment que certains établissements préfèrent abandonner la chirurgie pédiatrique à cette occasion. Pas assez rentable, trop de risques ajoutés par rapport à la chirurgie adulte.

«Ã‚ J’ai vu des familles subir quatre reports »

«Â On avait une forte diminution du nombre de chirurgiens pédiatriques formés, et on a un décret qui prévoit que les enfants soient opérés dans des plateaux techniques adaptés. On risque d’être confrontés à un problème d’accès aux soins », souligne la Pr Sabine Sarnacki (hôpital Necker), à l’occasion d’une séance dédiée au sujet à l’Académie de chirurgie. C’est parfois déjà le cas pour des « pathologies fonctionnelles, non vitales. J’ai vu des familles subir quatre reports, c’est de la maltraitance. »

«Ã‚ L’enfant fait peur »

«Â Il y a un manque de structures pour la chirurgie courante, qui ne devrait pas être réalisée dans les CHU », estime la Pre Christine Grapin-Degorno (hôpital Robert Debré). « À part la cardiologie, la chirurgie pédiatrique repose sur l’hôpital public, appuie la Pre Sarnacki. L’enfant fait peur et les parents sont de plus en plus exigeants. »

Environ 1 million d’enfants de moins de 15 ans sont opérés chaque année (12 % des opérations annuelles tous âges confondus). Certaines opérations, comme celles de la sphère ORL (végétations, amygdales, diabolos…) peuvent être réalisés par des ORL non spécialisés mais formés.
